1. General Use & No Doctor-Patient Relationship

This website is operated by CrossPointe Dental, Inc, a licensed Utah dental practice (Utah Code Title 58, Ch. 69), for informational purposes only. Content on this site does not constitute dental advice, diagnosis, or treatment, and does not establish a doctor-patient relationship. A formal patient relationship is only established upon an in-person visit.

2. Privacy & HIPAA Compliance

As a HIPAA-covered entity, we protect all health information in compliance with:

• HIPAA Privacy Rule (45 CFR Part 164, Subparts A & E) — governs use and disclosure of Protected Health Information (PHI)
• HIPAA Security Rule (45 CFR Part 164, Subpart C) — requires safeguards for electronically stored/transmitted PHI
• HIPAA Breach Notification Rule (45 CFR Part 164, Subpart D) — requires notification within 60 days of a breach

Our Notice of Privacy Practices (NPP) is available in our office and upon request, as required by 45 CFR § 164.520. You have the right to access your records under 45 CFR § 164.524. Online forms and scheduling tools on this site are HIPAA-compliant and use SSL encryption. Third-party vendors with PHI access are governed by Business Associate Agreements (45 CFR § 160.103).

3. Utah Privacy Law Compliance

We comply with the Utah Protection of Personal Information Act (Utah Code § 13-44-101 et seq.), which requires safeguards for personal data and breach notification for affected Utah residents. Breaches affecting 500+ residents require notification to the Utah Attorney General and Utah Cyber Center (Utah Code § 13-44-202). We also comply with the Utah Consumer Privacy Act (Utah Code § 13-61-101 et seq.) as applicable.

4. Website Accessibility (ADA)

We are committed to website accessibility for individuals with disabilities under Title III of the Americans with Disabilities Act, 42 U.S.C. §§ 12181–12189 (28 CFR Part 36). Contact us if you experience accessibility issues.

5. Children's Privacy (COPPA)

All website content is the property of CrossPointe Dental and protected by applicable copyright law. Testimonials and before/after photos represent individual results and are not a guarantee of outcomes. All advertising content complies with ADA (American Dental Association) guidelines and Utah dental board regulations.

7. Prohibited Use, Disclaimers & Limitation of Liability

You may not use this website for unlawful purposes, submit false information, attempt unauthorized access, or introduce malicious code. This website is provided “as is” without warranties of any kind. To the fullest extent permitted by law, we are not liable for any damages arising from your use of or reliance on this website.

8. Third-Party Links & Governing Law

We are not responsible for the content or privacy practices of third-party websites linked from this site. These Terms are governed by the laws of the State of Utah and applicable federal law. Disputes are subject to the jurisdiction of courts in Utah County, Utah. We may update these Terms at any time; continued use constitutes acceptance.

By consenting to SMS, you agree to these Terms. If you do not agree, please do not consent to the use of SMS.

1. SMS (Texting) Messaging Service

By providing your phone number, you consent to receive SMS text messages from CrossPointe Dental for appointment reminders, marketing messages, and general two-way communication. Message and data rates may apply. Reply HELP for support. Reply STOP to opt-out.

2. Payments

You may receive messages containing secure links to view and pay your account balance.

3. Frequency

You will receive 2–3 messages for appointment reminders and occasional texts to remind you to schedule or remit payment or to notify you of specials or services being offered.

4. Message and Data Rates

Message and data rates apply based on your mobile carrier's terms.

5. Privacy Policy

Your information will be handled in accordance with our Privacy Policy.

6. Opt-out Instructions

You can opt-out at any time by replying “STOP” to any SMS message. Reply HELP for support. You may also contact us directly at 801-224-7337 or office@uvdcare.com.

7. Liability

We are not responsible for any charges, errors, or delays in SMS delivery caused by your carrier or third-party services.

By opting in, you confirm that you are the owner or authorized user of the phone number provided and that you are at least 18 years old.
